(1) Rule 5b for those who wish to receive financial subsidy from the government of Nepal. Co-operative organizations that have been selected as BMOZAM should attach the following documents and details and submit them to the monitoring committee:-

(a) Rule 5b for shop operation. Evidence of being selected as per,
(b) three copies of details of the director of the cooperative organization with name, surname, address,
(c) Details of whether the shop has been started or not.
